1.1. The Swedish eHealth Agency, Sankt Eriksgatan 117, 113 43 Stockholm (the “Swedish eHealth Agency”) provides a service, Hälsa för mig, where You through a personal account have the possibility to collect, view and store information about Your health, daily life and healthcare. The purpose of Hälsa för mig ([hɛ:lsa fɶ:r mɛj], which translates to “Health for me”) is to offer You a tool for the collection, storing and sharing of health and healthcare related information, as well as for planning, implementation and follow-up of health and healthcare related activities and incidents.
1.3. You decide in Your sole discretion what information will be stored on Your account and what information that You may choose to share with Your family members or others. The information that you store on Your account may be of a very sensitive nature. You should therefore carefully consider whether or not to share Your information with others and You should not share Your information with others if You are not completely certain that You want to do so. The Swedish eHealth Agency will only maintain and technically process Your information on Your behalf. The Swedish eHealth Agency does not have the right to access the information that You have stored on Your account.
1.4. Hälsa för mig is not a medical record and no medical advice is provided through Hälsa för mig.
1.5. Your Hälsa för mig account is hereinafter referred to as the “Account”.
2. REGISTRATION AND TERMINATION OF THE ACCOUNT
2.1. In order to be able to register an Account, You must have a Swedish e-identification and You must be at least 18 years old. You may register an Account for Your own private purposes only.
2.2. You can register and sign in to Your Account by use of Your e-identification.
2.3. You are responsible for maintaining and using Your e-identification in a secure manner in order to avoid any unauthorized access to Your Account. You may not disclose Your e-identification to anyone so that it will be possible for somebody else to sign in to Your Account. You are responsible for all activities that occur in relation to Your Account through the use of Your e-identification.
2.4. The Account is free of charge. You are, however, responsible for any network traffic fees that may be charged by Your mobile operator or Internet service provider in connection with Your use of the Account. If You choose to connect services or applications to Your Account in accordance with Section 4 below, the provider of such services or applications may charge You fees.
2.5. You can terminate Your Account at any time. All information that You have stored on Your Account will then be erased. It is therefore important that you print, or save in Your own computer, any information that you wish to keep before You terminate Your Account. The Swedish eHealth Agency cannot retrieve any such information after You have terminated Your Account.
3. SHARING OF INFORMATION
3.1. As a general rule, You are the only person who has access to the information that You have stored on Your Account. However, You have the option of sharing Your information with other persons who have registered their own Hälsa för mig accounts by using a specific sharing feature. You decide in your sole discretion to what extent you would like to share Your information with other persons, as well as the rights to be granted to such persons with respect to Your information on Your Account.
3.2. If You choose to share Your information on Your Account with another person, You are responsible for the use of Your information by such person. If another person chooses to share his or her information with You, You are responsible for using and handling such information only in the manner permitted by such person. 3.3. You can at any time check and obtain an overview of the persons who have access to information on Your Account via the sharing feature. You can also at any time restrict or suspend the sharing of Your information with other persons.
4. USE OF APPLICATIONS
4.1. You can choose to use various services and features via Hälsa för mig by connecting applications provided by third parties (”Applications”) to Your Account. A list of available Applications is published on Hälsa för mig.
4.3. It is important to note that if You choose to share Your information on Your Account with another person and grant such person rights to your information, such person will also have the right to connect Applications, with the same rights, to Your information and to consent on your behalf that such Application may access and use Your information in accordance with Section 4.2. If another person chooses to grant such rights to You, You are responsible to obtain that persons consent before connecting an Application and grant such Application access to that person’s information.
4.4. If You have any questions regarding an Application or how Your information is used by an Application, please contact the provider of the Application. The Swedish eHealth Agency does not have any knowledge of, and is not responsible for, the functionality of Applications.
4.5. You can at any time check which Applications that You have connected to Your Account and what information You have chosen to share with the Applications. You can at any time discontinue the connection of an Application.
5. PERMITTED USE OF THE ACCOUNT
5.2. You acknowledge and agree: (i) to be responsible for all information that You collect and store on Your Account and to only store information that is necessary and relevant in order for You to use Hälsa för mig for the intended purposes; (ii) not to collect and store information on Your Account that is offensive, threatening, abusive, or entails slander, infringement of privacy rights, incitement to racial hatred, infringement of intellectual property rights, child pornography or any other unlawful information; (iii) not to copy, modify, introduce viruses or other malicious code or by any other means take any actions that may harm or affect the systems, software and security features that are used to provide Your Account.
5.3. If You discover that Your Account contains any such information, material or malicious code that is prohibited to store on Your Account in accordance with Section 5.2 above, You undertake to immediately erase all such information, material and malicious code.
5.4. If You discover that any third party is misusing or has misused Your Account or Hälsa för mig, You should notify the Swedish eHealth Agency immediately.
6.1. The Swedish eHealth Agency endeavours to make Hälsa för mig and Your Account available 24 hours a day, all year round, but cannot guarantee that Hälsa för mig and Your Account are always available, are free from defects or contain any specific functionality.
6.2. Hälsa för mig and Your Account are provided “as-is” and the Swedish eHealth Agency is not liable for any loss or damage arising from defects or unavailability of Hälsa för mig. The Swedish eHealth Agency reserves the right at any time to change, restrict or remove features and services that are available via Hälsa för mig and Your Account.
6.3. The Swedish eHealth Agency is not liable for any medical, financial or other decisions that You, or someone else, make that are based on the information on Your Account or based on Your use of Your Account. Further, the Swedish eHealth Agency is not liable for any loss or damage arising from an Application that You have connected to Your Account or arising from the sharing of Your information with another person.
7. PERSONAL DATA
7.1. The Swedish eHealth Agency is the data controller with respect to the processing of personal data in Hälsa för mig. The Swedish eHealth Agency will process Your personal data based on Your explicit consent. For information on how the Swedish eHealth Agency will process Your personal data, please see halsaformig.se under ”Processing of personal data”.
7.2. If You, or the person to whom You have granted such rights by use of the sharing feature, connect an Application to Your information and You have given Your explicit consent that such Application may access and use Your personal data, the recipient of Your personal data will be the data controller with respect to the subsequent processing of Your personal data in the Application.
